Job summary

The YMCA of Southern NC seeks a DPP Coordinator to lead the Diabetes Prevention Program across the region. You will oversee daily operations, facilitate lifestyle-change sessions, and supervise Lifestyle Coaches while coordinating outreach, enrollment, and grant reporting.

Travel to YMCA locations and partner sites throughout southeastern North Carolina is a regular requirement, with a grant-funded term through August 31, 2027.

Qualifications

• Bachelor’s degree in public health, health promotion, nutrition, exercise science, social work, education, human services, or related field preferred.
• Experience in health education, behavior change, wellness, chronic disease prevention, or community programming.
• Ability to coordinate multiple classes, locations, partnerships, deadlines, and participant needs.

Responsibilities

• Oversee regional delivery and daily operations of the DPP in line with CDC curriculum and grant requirements.
• Facilitate engaging lifestyle change sessions in person and via approved virtual formats.
• Recruit, screen, enroll, orient, and retain participants with ongoing encouragement and follow-up.
• Coordinate regional class schedules, locations, outreach, referrals, and communications.
• Supervise, train, schedule, and support lifestyle coaches to ensure program quality.
• Build relationships with health care providers, employers, and community partners.
• Maintain confidential participant records including attendance, weight, activity, and outcomes.
• Monitor enrollment and progress; use data to improve results.
• Prepare program and grant reports in partnership with leadership.
• Manage program supplies and assist with budgeting.
• Represent the YMCA at regional meetings and outreach events.
