This grant-funded position is authorized in one-year funding periods and is contingent upon continued grant funding, program requirements, and achievement of required outcomes. Grant funding may be renewed for additional periods; however, if funding is not renewed or otherwise becomes unavailable, employment in this position will end at the conclusion of the applicable funding period. Employment with Reid Health is at will, meaning that either the employee or Reid Health may end the employment relationship at any time, with or without notice and with or without cause, subject to applicable law.

Schedule

Monday through Friday 8:00 a.m.–5:00 p.m.; however, schedules may vary based on community events and departmental needs. Evening and weekend work will be required as needed.

About The Position

The Regional Community Health Worker (CHW) Coordinator provides operational coordination and administrative support for the Region 4 Community Health Worker Network under the direction of the Manager. This position supports the development and sustainability of a connected regional CHW infrastructure by facilitating communication, collaboration, resource sharing, and alignment among healthcare organizations, community-based organizations, public health agencies, and other network partners. The Regional CHW Coordinator serves as a central point of coordination for network activities, helping to organize meetings, maintain communication channels, support training and professional development opportunities, manage shared resources, and promote standardized practices across the region. The role strengthens collaboration, reduces duplication of effort, and supports consistent, high-quality CHW services by maintaining coordinated referral pathways, shared tools, and ongoing engagement among Community Health Workers in Blackford, Fayette, Grant, Henry, Jay, Madison, Randolph, Tipton, Union, and Wayne Counties.

Overview Of Responsibilities

Regional Network Coordination

• Coordinates day-to-day operations of the Region 4 Community Health Worker (CHW) Network under the direction of the Community Health Worker Manager and Director.
• Organizes and facilitates network meetings; including speakers and trainings, workgroups, learning collaboratives, and special events; prepares agendas, distributes materials, documents outcomes, secures speakers, venues, day of needs and tracks follow-up activities.
• Supports regional CHW workforce development through hands on training, onboarding, certification opportunities, continuing education activities, and professional development resources for all network partners as needed.
• Promotes alignment of CHW practices, documentation standards, workflows, and referral processes across participating organizations.
• Develops training plans for CHWS to be shared with network partners.
• Assists with special projects and initiatives that advance CHW workforce development, care coordination, and community health improvement efforts.

Communication and Resource Management

• Develops and maintains ongoing communication with network members through newsletters, announcements, email communications, and other engagement platforms.
• Creates, maintains, and promotes shared resource platforms containing training materials, referral tools, onboarding resources, best practices, and community resource information.
• Collects, organizes, and disseminates resources, tools, and information contributed by network partners to support shared learning and replication of successful practices.
• Facilitates information exchange regarding community resources, program updates, policy changes, and best practices relevant to CHWs.

Partnership Development and Community Engagement

• Establishes and maintains collaborative relationships with Community Health Workers, healthcare organizations, community-based organizations, public health agencies, referral partners, and regional stakeholders.
• Assists with community outreach and engagement efforts by coordinating events, managing logistics, collaborating with community partners, supporting staff participation, distributing educational and outreach materials, and tracking outreach activities and participant engagement.
• Serves as an active participant in the Region 4 CHW Network by strengthening referral pathways, identifying service gaps, supporting coordinated resource navigation efforts, and promoting regional collaboration.
• Attends community meetings, coalitions, workshops, and partner convenings to strengthen partnerships, identify emerging community needs, expand resource networks, and improve access to community services.
• Conduct blood pressure and glucose screenings at community events when qualified and trained to do so.
• Follow established community screening policies and procedures, maintain required equipment and supplies, and complete required training and competency requirements.
• Plan, coordinate, and participate in community outreach events and health fairs, including setting up and managing vendor tables, engaging attendees, sharing health and community resources, gathering feedback, and supporting efforts to increase awareness of organizational programs and services on an as needed basis.

Data, Reporting, and Network Evaluation

• Supports data collection, reporting, grant activities, program evaluation, and continuous improvement efforts related to network goals and objectives.
• Maintains accurate records of meetings; including agendas, meeting minutes, attendance, communications, training activities, resource inventories, and network participation.
• Monitors network engagement and participation trends, identifies emerging needs, and communicates opportunities for improvement to network leadership.
• Assists with regional asset mapping activities, including identification of CHWs, community resources, partnerships, and service gaps to reduce duplication and strengthen network capacity.

Community Health Support Activities

• Assists with monthly food distribution activities and other community health initiatives as needed.
• Supports onboarding and training activities for new Community Health Worker team members.
• Plays an active role in evaluating and developing new processes related to transportation and other resource support workflows.

Professional Development

• Maintains required Community Health Worker certification and completes all continuing education requirements in accordance with organizational and state standards.

Education & Experience

• Education Required: High school diploma or equivalent
• Education Preferred: Bachelor’s degree in applicable field
• Experience Required: Minimum of 2 years experience with a hospital, health agency, or community service organization with demonstrated communication and interpersonal skills with the ability to engage effectively with individuals from diverse cultural and socioeconomic backgrounds. Knowledge of community resources and social support services available to individuals and families.
• Experience Preferred: Knowledge and experience in the areas of community health, social services, healthcare settings

Licensure

• Certified Community Health Worker (Indiana) preferred upon hire or the ability to obtain within 60 days of hire
• Community Health Worker continuing education requirements must be maintained through completion and tracking of approved CEU opportunities.
