Job summary

Reid Health is seeking a Regional Community Health Worker (CHW) Coordinator to support the Region 4 CHW Network and strengthen collaboration among healthcare organizations and community partners.

You will coordinate trainings, communications, and resource sharing, while promoting standardized CHW practices and referral pathways across multiple counties. A focus on workforce development and community engagement is essential.

Qualifications

• Education Required: High school diploma or equivalent.
• Education Preferred: Bachelor’s degree in applicable field.
• Experience Required: Minimum of 2 years in hospital, health agency, or community service with strong communication and interpersonal skills.

Responsibilities

• Coordinate day-to-day operations of Region 4 CHW Network under CHW Manager and Director.
• Organize and facilitate network meetings, trainings, and events; prepare materials and track follow-up.
• Support CHW workforce development through onboarding, training, and continuing education.
• Develop training plans for CHWs to share with partners; promote standardized practices.
• Plan and participate in community outreach events, health fairs, and screenings when trained.
