Job summary

UNC Health Rex and UNC Health Johnston seek a Regional Care Management leader to oversee operations, guide department leadership, and engage stakeholders in care coordination and outcomes management. You will oversee People Management, Fiscal Management, and Quality initiatives to ensure high-quality care aligned with organizational standards.

Responsibilities include building partnerships, improving patient outcomes, managing budgets, and leading performance metrics across the regional care

Qualifications

• Master's degree in Nursing, MSW, or related field.
• Valid NC RN license or LCSW/LCSW-A to practice in NC within three years of hire.
• ACM or CCM certification within two years of hire.
• Seven years of experience in Care Management; two years in healthcare management.

Responsibilities

• Communication – develops and maintains positive relationships with internal and external stakeholders.
• Customer Service – improve customer satisfaction and care management processes.
• Fiscal Management – assist in budgeting and resource management for regional departments.
• Goals/Outcomes Management – monitor departmental performance and implement improvements.
• Quality & Compliance – lead quality improvement and ensure regulatory compliance.
