Job summary

Kindred Hospital Las Vegas - Sahara is seeking a dedicated care coordinator to manage patient populations across the care continuum. You will collaborate with the ICT team to ensure optimal resource use, care delivery, and compliance with external standards.

Responsibilities include coordinating clinical and psychosocial activities, assisting with care transitions, and advocating for patients and families to support informed decisions.

Qualifications

• Postsecondary nursing cert/diploma from an accredited school is required.
• BSN/MSN/BSW/MSW preferred.
• RN license or LCSW/CSWCM eligibility on hire; CSWCM preferred.
• Healthcare experience (2+ years) required; case management or discharge planning preferred.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Monitors all areas of patients’ stay for effective care coordination.
• Appropriately refers high risk patients who would benefit from additional support.
• Serves as a patient advocate and enhances collaboration for informed decisions.
• Participates in interdisciplinary rounds to review goals and optimize resource utilization.
• Coordinate care planning and discharge planning across the care continuum.
