Job summary

In this role, you will coordinate essential services for patients in long-term care and skilled nursing facilities. You’ll be responsible for ensuring seamless service delivery while collaborating with various healthcare professionals.

This position requires travel within your assigned region and offers a mix of office and clinical environments. You will also manage documentation and utilize electronic devices effectively, making strong tech skills essential for success.

Qualifications

• Minimum of 3-5 years of experience in skilled nursing facilities.
• Proficient in using a PC in a Windows environment.
• Must be vaccinated against COVID-19.

Responsibilities

• Travel to locations within assigned region for support and services.
• Engage with facility staff for effective coordination.
• Facilitate service coordination including Psychiatry and Therapy.
