Job summary

The organization seeks a Patient Referral Liaison to connect patients with providers and health resources. You will interpret between languages, coordinate referrals, and document care activities to support continuity of care for families.

You will inform patients about appointments, follow up with specialists, and ensure HIPAA compliance in all communications. Strong organizational skills, cultural sensitivity, and the ability to work with minimal supervision are essential.

Qualifications

  • Preferred associate degree or higher.
  • Experience with healthcare referrals preferred.
  • Preferred holder of interpreting certificate.
  • Willingness to work with all cultural and socioeconomic groups without judgment or bias.
  • Demonstrates ability to cooperatively work/mediate with all age groups and family groups.
  • Compliance with HIPAA law and regulation; ability to confidentially retain information.
  • Ability to work with minimal supervision and exercise sound independent judgment.
  • Excellent familiarity and application with medical terminology.

Responsibilities

  • Collects and evaluates information about a patient to assist in achieving patient/family needs and realistic outcomes.
  • Interprets between foreign language and English to facilitate health care service.
  • Refers and coordinates appropriate processes as assigned.
  • Researches, documents and informs coworkers and patients about available health resources at local, state and federal levels.
  • Notifies patients of appointments and conducts follow-ups to ensure attendance; reminds providers to submit reports.
  • Maintains competency in inputting medical information into clinical information systems and completing referrals.
  • Documents actions taken and activities performed clearly and accurately.
  • Provides continuity of care to each patient and their family members.
  • Performs other duties as assigned.
