Job summary

Essential Services LLC is seeking a Referral Specialist for a full-time, in-office role. M-F, 9:00 AM–5:00 PM, in McMinnville, OR, focusing on referral intake, data accuracy, and client coordination to determine service fit for individuals with IDD.

You will be the first contact for clients, families, Case Managers, and brokerages, handling information gathering, meetings, and handoffs to Service Coordination while supporting a collaborative team environment and adherence to processes.

Qualifications

  • At least 21 years of age.
  • Able to pass a DHS criminal history check.
  • Legally eligible to work in the United States.
  • No substantiated abuse or neglect findings.
  • Not listed on CMS excluded or debarred providers list.
  • Not the spouse of a client you would directly work with.
  • Valid driver’s license and auto insurance (if applicable to duties).

Responsibilities

  • Receive and respond to incoming referrals and service inquiries.
  • Gather key information regarding prospective client needs, authorized services, and preferences.
  • Maintain accurate Prospective Client Logs and referral records.
  • Track referral communications and follow up on outstanding information.
  • Determine whether requested services align with the support Essential Services LLC can provide.
  • Serve as an initial point of contact for prospective clients, families, Case Managers, and Brokerages.
  • Schedule and attend Prospective Client Meetings.
  • Communicate referral status and next steps to appropriate parties.
  • Provide a welcoming and informative experience throughout the referral process.
  • Identify when a prospective client is ready to move forward with services.
  • Provide Service Coordinators with complete and accurate referral information.
  • Communicate important client needs, preferences, and service details during the handoff.
  • Support a smooth transition from referral to Service Coordination.
  • Participate in the rotation of the Emergency After-Hours Phone.
  • Attend client and DSP events as needed.
  • Maintain organized and accurate records.
  • Complete additional referral and client support tasks as assigned.

Description

At Essential Services LLC, we support individuals with intellectual and developmental disabilities by connecting them with qualified Direct Support Professionals (DSPs) who provide in-home and community-based care. Our team works together to provide consistent, responsive, and individualized support to the clients we serve.

About the Role

This is a full-time, in-office position, Monday through Friday from 9:00 AM to 5:00 PM.

As a Referral Specialist, you will manage incoming referrals and serve as an initial point of contact for prospective clients, families, Case Managers, and Brokerages. You will gather key information, maintain accurate referral records, coordinate prospective client meetings, and help determine whether Essential Services LLC can meet the individual’s service needs.

Key Responsibilities
1. Referral Management
  • Receive and respond to incoming referrals and service inquiries
