Job summary

Petaluma Health Center seeks an entry-level referrals coordinator to ensure patients access specialty services by coordinating insurance authorizations and referrals. You will verify coverage, assist with PAP enrollment for uninsured patients, and maintain accurate referral tracking to support timely care.

Qualifications

• High school diploma with medical office training.
• Two years experience with insurance, referrals, authorizations, and community contacts.
• Ability to communicate effectively in English and Spanish.

Responsibilities

• Verify patient insurance eligibility.
• Maintain updated authorizations/referrals information in patient chart through a tracking mechanism.
• Provide assistance to uninsured patients to be enrolled in PAP program.
• Provide information to patient through warm hand offs as needed.
• Process all outside referrals from other PCP/specialist within expected timeframe.
• Track referrals by using structured data to maintain up to date accurate information.
