Job summary

Behavioral Health Connections, Inc. provides patient-focused mental health services and 24/7 crisis support. We are seeking a Referral Management RN for PRN varied 12-hour shifts to manage referrals, coordinate care, and review medical records.

You will support crisis assessors and ensure HIPAA compliance while working with sending facilities, patients, and families. The role requires an active RN license, current CPR, and experience in behavioral health, with responsibilities including

Qualifications

• Licensed RN with current state nursing license required.
• CPR certification and vaccines as required by customer hospital.
• 1–3 years of experience in behavioral health preferred.

Responsibilities

• Review assessment referrals received from various sources in collaboration with the RN and other clinicians to assess patient needs.
• Assist in coordinating patient care.
• Maintain patient records, update databases, and ensure accurate documentation of patient information in approved EMR and other platforms.
