Job summary

Genesis Healthcare is seeking a Post-Acute Services Director to drive growth and ensure smooth patient referrals and transitions across nursing centers. You will manage discharge processes, build hospital and non-hospital relationships, and implement scalable sales and clinical programming strategies to sustain market presence.

Ideal candidates will have RN/LPN licensure, a valid driver’s license, and 5–7 years of recent clinical experience.

Qualifications

• RN or LPN licensure preferred, but not required, except where mandated in the state in which employment occurs.
• Valid driver’s license.
• Five to seven years of recent clinical experience.

Responsibilities

• Lead the patient referral process from review through to acceptance, ensuring smooth admissions and proper clinical handoffs.
• Develop and sustain a market strategy plan for assigned nursing centers with data analytics to drive admissions and sales goals.
• Establish and cultivate relationships with hospital referral sources and key decision-makers to generate high-quality referrals.
