Genesis HealthCare, Inc. seeks a Post-Acute Services Director to drive the growth of assigned nursing centers by cultivating hospital referrals and managing patient transitions.

You will oversee discharge processes and implement sales and clinical programming strategies to expand the center’s market presence. The role focuses on building hospital relationships, using CRM for seamless handoffs, and delivering consistent referral conversions while meeting admissions goals and improving operational

Qualifications

• RN or LPN licensure preferred, but not required where state mandates.
• Five to seven years of recent clinical experience.
• Valid driver’s license required.

Responsibilities

• Lead the patient referral process from review to admission using CRM to document handoffs.
• Develop 5–10 new hospital referrals monthly by building relationships with key decision-makers.
• Articulate the clinical strengths of the centers and align offerings with hospital referral needs.
• Achieve a 60%+ conversion rate for clinically appropriate referrals and drive improvements over 90–120 days.
• Create and execute a market strategy plan with data analytics to exceed admission targets by at least 5% each period.
