The Homecare Intake & Referral Development Coordinator serves as the first point of contact for prospective clients, families, hospitals, rehabilitation facilities, physician practices, case managers, social workers, and community referral partners. This role combines intake coordination with business development responsibilities to drive agency growth through referral conversion and relationship management.
Serve as the primary contact for referrals and inquiries; build referral relationships; conduct outreach; educate referral sources; participate in community events and maintain referral databases.
Respond promptly to inquiries, assess needs, present services, guide enrollment, convert referrals into admissions, and follow up consistently.
Verify insurance eligibility, coordinate authorizations, communicate benefits, and secure required documentation.
Deliver exceptional customer service while supporting seamless transitions into home care services.
Maintain compliant records, HIPAA standards, and referral/admissions reporting.
2+ years in healthcare sales, intake, admissions, business development, or referral management. Home care experience preferred.
Consultative selling, relationship management, communication, organization, CRM/EMR proficiency, and customer service excellence.
Referral volume growth, conversion rates, admissions achieved, referral source development, census growth contribution, response times, and customer satisfaction.
