Job summary

HHM Health in Mesquite, TX is seeking a Referral Coordinator to join our on-site team. You will process medical, dental, and specialty referrals and PAP applications, ensuring timely access to care for our patients while maintaining high service standards.

Key responsibilities include tracking referrals, coordinating with providers and insurance, updating patients, and maintaining clear documentation to support safe, coordinated care.

Qualifications

• High School Diploma or equivalent.
• Experience as Referral Coordinator or similar in healthcare settings.
• Experience with community health centers and/or non-profits preferred.
• Detail-oriented and able to multitask in high-volume environments.
• Strong written and verbal communication skills.
• Experience serving multicultural and multilingual patient populations.
• Experience with eClinicalWorks or similar and MS Office Suite.
• Bilingual in Spanish, Pashto, Burmese, Rohingya or other languages.

Responsibilities

• Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and patient safety.
• Update providers on issues or concerns regarding referrals and communicate directly with patients and providers.
• Work with insurance companies and care providers to share essential patient information, assist in scheduling appointments, and manage health insurance issues.

• Work and track outgoing referrals by urgency and requested timeline.

About the Role

We are seeking a Referral Coordinator with a passion for service excellence to join our team in the Mesquite area. You will help expand our impact on the community by providing access to high quality, compassionate health care. The Referral Coordinator is responsible for processing medical, dental, and specialty referrals, as well as PAP applications for patients of HHM Health.

Location

4550 Gus Thomasson Rd, Ste. 40, Mesquite, TX 75150 (100% On‑site role)
