Job summary

Qualifications

Responsibilities

• Review and prioritize incoming referral requests per standards.
• Process referrals daily by phone, fax, or email to secure appointments.
• Gather clinical information and forward to specialists with timeliness.
• Document communications and track referrals in the referral log.
• Communicate with patients about appointments and care coordination.
