Job summary

The Veterans Health Administration is seeking a Referral Coordination Team Registered Nurse in Columbia, SC. You will serve as liaison to internal and community providers and manage Veteran care throughout the consult process.

You will assess, plan, implement, and evaluate age-appropriate care, coordinate education and self-management, and collaborate with the interdisciplinary team to improve access and timely referrals to community or VA care.

Qualifications

• Must have active RN license in any US state or territory.
• Experience coordinating care across VA and community providers.
• Strong ability to educate patients and coordinate self-management.
