Job summary

LeadingAge Virginia is seeking a Referral Coordination Team Program Manager RN to lead and optimize care coordination for Veterans across the continuum of care. This role requires strong leadership, program management, and clinical excellence to improve safety and outcomes.

You will plan, organize, direct, and evaluate the RCT program and staff, ensuring compliance with VA standards and effective collaboration with internal providers and community partners.

Qualifications

• Active RN license in the United States.
• Eligible for licensure and appointment in VA or equivalent state license.
• English language proficiency required for direct patient care.
• BSN preferred; ADN acceptable with appropriate experience.

Responsibilities

• Lead the Referral Coordination Team Nurses to coordinate Veteran care across the consult process.
• Ensure compliance with regulations and VA guidelines; drive process improvement through data analysis.
• Collaborate with providers and internal staff; serve as SME for relevant medical policies and RCT processes.
