Job summary

Qualifications

• Knowledge and skills necessary to provide patient assistance with the referral process.
• Empathy and sensitivity necessary to elicit patient cooperation.
• Ability to run a copy and fax machine.

Responsibilities

• Makes referrals for patients to other physicians or testing.
• Coordinates with resources to assure continuity of care.
• Documents referrals in the Electronic Health Record (EHR).
