Job summary

Arcadia Home Care in California is seeking a referrals and admissions coordinator responsible for coordinating the referral and admissions process, working with facility representatives, discharge planners, and other agencies to initiate care.

The role requires at least 1 year in home care sales, excellent interpersonal, verbal and written communication, and the ability to manage documentation, schedules and deadlines while maintaining compliance.

Qualifications

• Be eighteen years of age or older.
• At least one year experience in homecare, hospice and/or healthcare field preferred.
• Ability to complete necessary documentation accurately, legibly and timely.
• Ability to handle stressful and delicate situations.
• Strong interpersonal, verbal and written communication skills.

Responsibilities

• Coordinate referral and admissions process with facilities, discharge planners and agencies.
• Oversee scheduling of initial nursing assessment and admission.
• Maintain admission packets and ensure forms are available to admissions staff.
• Provide telephone and clerical support to staff as time allows.
• Maintain accurate clinical records and track client goals and progress.
• Adhere to regulations, policies, procedures and standards.
• Assist with marketing activities to promote relations with referral sources.
