Job summary

The Osborne Association's ReentryWorks unit seeks a Transitional Planner I in Newburgh to coordinate reentry services, assess needs, and connect participants with community resources prior to release from NYS prisons.

This hybrid role requires transporting participants to NYC, arranging release-day support, and collaborating with DOCCS and service providers. A valid NY driver’s license and willingness to travel Hudson Valley are essential; Spanish language skills are preferred.

Qualifications

  • High School Diploma or equivalent with at least one year of social service, case management, or customer service experience.
  • Valid New York State Driver’s License and clean driving record.
  • Bilingual Spanish is preferred and helpful for participant communication.

Responsibilities

  • Coordinate with service providers and DOCCS to finalize transitional plans and referrals.
  • Draft comprehensive transitional plans for participants.
  • On release day, assist with logistics and transportation to NYC.
  • Transport participants to residences or programs in NYC.
  • Provide transportation of participants, staff, or materials as needed.
  • Provide motivational interviewing with participants when needed.
  • Manage the ReentryWorks vehicle and arrange maintenance.
  • In-person collaboration with coworkers is required.
  • Schedule is subject to change; perform other duties as assigned.
