Job summary

The Osborne Association in New York is looking for a Case Manager for the Fulton Reentry department. This role will provide case management services to homeless individuals, ensuring proper documentation and coordination with other services.

Qualifications

• Associates Degree in Human Services, Sociology or related field with one year of case management experience.
• In lieu of a degree, High School Diploma with two additional years of case management experience.
• Proven ability to work collaboratively with diverse groups.

Responsibilities

• Provide direct client services including intake, engagement, assessment and service plan development.
• Manage a caseload for a diverse client population.
• Document all interactions with clients and maintain client progress.
