Job summary

The Department of Corrections seeks a Psychiatric Social Worker 3 (PSW3) for the Washington Corrections Center in Shelton, WA. This full-time day shift role supports transition planning, medical and behavioral health services, and coordination with community resources to aid release.

You will conduct reentry assessments, develop Continuity of Care Plans, coordinate with DDA, HCS, and MCO, and provide Narcan education.

Qualifications

• Must be able to obtain and maintain status as an Agency Affiliated Counselor through the Washington State Department of Health or possess a current, valid WA Social Work license.
• Incumbent must apply for agency affiliated counselor license within 7 days of hire; may provide services within the agency during processing.
• Must have a current, valid Driver’s License.

Responsibilities

• Conduct release planning for incarcerated individuals who have healthcare follow up needs and identify transition needs.
• Complete Reentry Needs Assessment and Health Services Reentry Plan.
• Coordinate with DDA and HCS and with MCO for post-release services.
• Provide Narcan administration education.
• Document all contacts with patients per policy and input OMNI encounters.
