Job summary

Qualifications

Responsibilities

• Assess problems and capacities; discuss case planning and monitor progress.
• Provide counseling and develop referrals to community resources upon transfer or release.
• Coordinate multidisciplinary treatment with staff, parole agents, and community resources.
