Job summary

Region Ten Community Services Board seeks a dedicated Case Manager to provide comprehensive case management for individuals with mental illness, emotional disturbance, substance use, or developmental disabilities. You will assess needs, develop access plans, and liaise with clients, families, programs, and providers to advocate and educate the community.

You will report to the Director III for Louisa County Clinic and work with initiative and independent judgment under supervision.

Qualifications

• Bachelor’s Degree preferred.
• Degree in Human Services or related preferred.
• Valid Virginia driver's license and acceptable driving record required.

Responsibilities

• Enhance community integration by increasing access, promoting community living skills and using local resources.
• Make collateral contacts to promote implementation of the individualized services plan.
• Assess needs and plan services, developing an individualized services plan (ISP).
• Link individuals to community supports to achieve life goals in the ISP.
• Assist individuals to locate or obtain needed services and benefits.
• Coordinate services and planning within the agency and with other providers and systems.
• Monitor service delivery through contacts, site and home visits, and quality assessments.
• Provide follow-up education and counseling to support the ISP.
• Advocate for individuals as plans evolve.
• Develop crisis plans and provide emergency prescreening as needed.
• Plan transitions in individuals’ lives and coordinate health care needs.
