Job summary

A community health organization is seeking a Medical Case Worker II to support Medi-Cal–eligible adults involved in the justice system in Los Angeles. This role is responsible for providing release planning services and coordinating health services for individuals transitioning from incarceration. The ideal candidate will have a bachelor’s degree and experience in casework, as well as the ability to work in correctional health facilities. Additional qualifications include successful completion of applicable clearances and good judgment in stressful situations.

Qualifications

  • Clear Health Clearance required.

Job description

The Medical Case Worker II supports Medi-Cal–eligible adults involved in the justice system as part of California’s Medicaid 1115 JI Reentry Initiative. This role supports individuals transitioning from incarceration in state prisons, county jails, back into the community. The Medical Case Worker II works with clients during incarceration and post-release to ensure continuity of Medi-Cal coverage, access to comprehensive health services, medications, and supportive resources, including linkages to Enhanced Care Management (ECM). The position plays a critical role in improving health outcomes, supporting successful reentry, and reducing recidivism through coordinated, client-centered care.

ESSENTIAL FUNCTIONS
  • Provides release planning services to clients with forensic history, mental illness and/or dual diagnosis.
  • Receives and reviews referrals for release planning from an electronic generated list; to identify release planning needs and identifying information.
  • Meets with clients to introduce and explain program services and offer release planning services.
  • Utilizes applications and databases to research client historical information, location of client, court date, housing, health status and level of observation.
  • Makes referrals and works with various county departments and community programs, such as Department of Mental Health, Public Defender, Public Guardian, Housing for Health, Justice Care and Opportunities, and various substance use treatment providers.
  • Facilitates interviews with community providers for program placements and coordinates releases to programs.
  • Collaborates with jail entities to facilitate program placement and client release.
  • Documents client encounters and finalization of client’s release plan in client’s medical chart.
  • Updates team shared client tracking spreadsheets.
  • Uploads documents to team’s folder.
  • Places resources in client’s property
  • Assists program development in training and orienting onboarding staff.
  • Completes DHS-CHS department trainings in a timely manner.
  • May act as team lead, reviewing team assignments, and triaging assignments to team members.
  • Maintains a consistent level of face‑to‑face contact and care coordination, delivering timely services using PRCM workflows.
  • Assists PRCM teams as needed.
  • Assists with other tasks as needed.
JOB QUALIFICATIONS

Ability to follow oral and written instructions. Ability to exercise sound judgement while working in a stressful environment. Able to lift up to a 10-pound limit and some bending, stooping, or squatting.

Education/Experience
  • One (1) year experience as a Medical Case Worker

OR

  • A bachelor’s degree from an accredited college or university and one (1) year of professional casework experience interviewing, counseling, and assisting patients/clients.
Certificates/Licenses/Clearances
  • Successfully clear the LA County Live Scan process.
  • Successfully clear the Health Clearance.
