Job summary

West State Street is hiring an Activity Assistant in a long-term care setting. You will plan, organize, and implement engagement activities to meet the physical, creative, cognitive, social, and spiritual needs of our patients and residents.

You will document attendance, participate in monthly recreation therapy planning, and attend care plan meetings as needed. The role emphasizes clear communication, strong organization, and compassionate interaction with residents, families, and staff.

Qualifications

• High school diploma or equivalent.
• Must be able to read, write, speak and understand English to ensure safety and care needs.

Responsibilities

• Plan, organize and implement engagement activities for patients and residents.
• Provide input into monthly recreation therapy service design.
• Document attendance and participation in recreation programs.
• Attend care plan meetings as needed.
