Responsibilities

• Identify patient entry into services and assess needs.
• Provide advocacy, education, recovery and wellness support.
• Engage patients in discharge planning and follow-up.
• Document all discharge planning components in EMR accurately.
• Coordinate care with the interdisciplinary team and ensure warm handoffs.
• Support transportation and access barriers as needed.
• Provide recovery support post-discharge and liaise with insurance programs.
