Health Here seeks a Certified Recovery Support Specialist to provide compassionate, recovery-focused support services to patients. You will engage clients, coordinate discharge plans, document in EMR, and collaborate with the care team to promote wellness.

Qualifications

  • Certified Peer Recovery Specialist (CPRS) within 3 years, with Narcan training within the first month.
  • Lived recovery experience.
  • Advanced reading, writing, and oral communication skills.
  • Ability to respond appropriately to diverse interactions, maintain confidentiality, and project professionalism.
  • Basic computer proficiency with familiarity in Windows OS, word processing, spreadsheets, email, and web browsing.

Responsibilities

  • Ensure comprehensive patient services, including assessment, discharge planning, and warm handoff.
  • Provide advocacy, education, and recovery support, especially for patients in crisis.
  • Engage patients within the living room process and establish collaborative relationships.
  • Assist in creating and facilitating recovery and discharge plans, documenting in EMR.
  • Support ongoing treatment engagement, including post-discharge follow-up and coordination with community services.
  • Educate patients and families on addiction, overdose signs, naloxone use, and treatment options.
  • Collaborate with the care team, ensure proper documentation, and participate in training and compliance standards.
