Boston Medical Center (BMC) is seeking a Recovery Support Navigator to provide care management and navigation for patients in the substance use treatment system. You will coordinate referrals, assist with insurance navigation, and support access to therapy and primary care within Project ASSERT.

The role emphasizes teamwork with a multidisciplinary health care team, cultural sensitivity, and strong documentation in the EMR.

Qualifications

  • Bachelor’s degree (or equivalent) preferred.
  • Degree in social work, psychology or related human services field is highly preferred.
  • Associate’s degree (or equivalent combination) plus a minimum of 1-2 years in healthcare is preferred.

Responsibilities

  • Document in the electronic medical record and ensure data entry, data integrity, and data tracking compliance including billing.
  • Screen patients for substance use and determine level of care using TAPs and other screeners.
  • Provide care management and navigation to engage patients and access treatment services and community resources.
  • Assist patients in locating and establishing substance use treatment services and coordinating referrals to primary care, withdrawal management, and therapy.
  • Monitor patient progress with functioning, adherence to treatment plans, and service needs.
  • Schedule outpatient visits and follow-up as needed.
  • Educate and support clients with culturally and linguistically appropriate strategies; coordinate with Interpreter Services.
  • Build relationships with community resources to ensure patient access and promote collaboration among the care team.
  • Document encounters and interventions with date/time/signature in EMR; bill for encounters accordingly.
  • Maintain compliance with policies, procedures, and data reporting requirements; adapt to departmental needs.

Essential Responsibilities / Duties
  • Responsible for documentation in the electronic medical record and complying with all data entry, data integrity, and data tracking requirements, and billing for services.
  • Screen patients for substance use using TAPs and other screeners to determine level of care.
  • Recovery Support Navigators (RSN) provide care management and navigation support to engage patients in care and help them access treatment services and community resources.
  • RSN assist members in locating and establishing substance use treatment services, coordinating referrals to primary care, withdrawal management, clinical stabilization services, and short- and long-term care, including therapy, while navigating insurance issues.
  • Monitors patients’ progress via patient’s level of functioning, adherence to treatment plans, recovery/relapse process and/or service needs.
  • Assists with scheduling outpatient visits and follow-up as needed.
  • Provides education, counsels, and supports clients. Uses a variety of culturally, linguistically and educationally appropriate strategies, in a variety of settings. Coordinates with Interpreter Services, as needed.
  • Develops and maintains strong relationships with the community and resources to ensure patient access.
  • Collaborates and coordinates with multi-disciplinary health care team members to promote positive prevention, harm reduction.
  • Documents patient encounters, resource development efforts, and other interventions for each patient, including date, time, and signature in the electronic medical record.
  • Bills appropriately for each patient encounter.
  • Complies with departmental, regulatory and contract requirements for documentation recording, and data collection, and other records and reports within required time frames.
  • Provides other administrative and special project assistance as required or assigned.
  • Remains knowledgeable of and follows appropriate policies, procedures, and work standards, including but not limited to, policy regarding hours of work, absenteeism, and tardiness.
  • Maintains competency in areas of responsibility to ensure quality of care
  • Maintains strict protocols of all confidential or sensitive information
  • Attends hospital required and relevant training sessions or activities.
  • Promotes a positive and collaborative work environment supportive of the hospital’s missions and goals.
  • Adapts to changing departmental needs, including but not limited to, offering assistance to other team members, adjusting assignments, and other functions.
  • Performs other duties assigned or as necessary.
Education
JOB REQUIREMENTS
  • Bachelor’s degree (or equivalent) preferred.
  • Degree in social work, psychology or related human services field is highly preferred.
  • May waive experience requirement for candidates with Bachelor's degree or higher.
  • Associate's degree (or equivalent combination of formal education and experience in community health) plus a minimum of 1 - 2 years in healthcare is preferred.
Certificates, Licenses, Registrations Preferred
  • Licensed Alcohol and Drug Counselor (LADC): not required, but encouraged to apply.
Experience
  • Experience in a healthcare setting is preferred, especially working with individuals who have substance use disorders and other complex needs.
Knowledge And Skills
  • Knowledge of community, social, and health resources, particularly substance use-related resources.
  • Empathic understanding of patients and skilled in performing multicultural needs assessments and developing concrete service plans.
  • Expertise in establishing working relationships with patients, their family, professionals, and the community, including the ability to collaborate with a multidisciplinary health care team.
  • Computer competency, inclusive of the ability to access and enter data.
  • Ability to work in a fast paced environment, inclusive of organizational skills, critical thinking, and problem solving abilities.
  • Effective verbal, written, and interpersonal communication skills to facilitate interactions with various members of the health care team, patients, and their families.
  • Bilingual or multi-lingual skills (beyond that of English) appropriate to the patient population served, is preferred.
  • Demonstrated flexibility with client’s priorities, evolving needs, and goals.
  • Sensitivity to ethnic, cultural, gender, and sexual orientation diversity; values; beliefs; and behaviors
  • Demonstrated critical thinking and problem solving ability
Compensation Range

$22.84- $31.97

This range offers an estimate based on the minimum job qualifications. However, our approach to determining base pay is comprehensive, and a broad range of factors is considered when making an offer. This includes education, experience, skills, and certifications/licensures as they directly relate to position requirements; as well as business/organizational needs, internal equity, and market-competitiveness.
