Job summary

Child & Family Services, Inc. is seeking a Recovery Support Navigator to assist individuals on their recovery journey. You will screen, intake, obtain insurance authorizations, and refer clients to services, working closely with recovery coaches.

You will provide trauma-informed, person-centered care, help explore treatment options, coordinate with the care team, and support transportation needs. RSN services run seven days a week, including on-call and mobile relief when appropriate.

  • Collaborate with the care team to set treatment goals in the care plan.
  • Assist with transportation to essential medical and behavioral health appointments.
  • Deliver RSN services seven days per week, including on-call coverage.
  • Provide mobile services to consumers in safe settings.
  • Maintain professional relationships with community resources and partners.
  • Assist with referrals to primary care, mental health, addiction treatment, and housing supports.
  • Stay knowledgeable about the substance use disorder service system.
  • Maintain confidentiality and follow 42-CFR and privacy rules.
  • Participate in supervision and training activities as required.

Summary

The Recovery Support Navigator will provide support to individuals in their recovery journey. The RSN will work closely with recovery coaches and be responsible for screening, intakes, insurance authorizations, and making appropriate referrals for services.


FULL-TIME SCHEDULE: Monday 8AM-4:30PM, Tuesday 11:30AM-8PM, Wednesday 10AM-6PM, Thursday 11:30AM-8PM, Friday 8AM-4:30PM

Job Responsibilities & Essential Functions


  • Conduct screening and intake interviews to determine consumer eligibility for services

  • Provide trauma-informed, recovery-oriented, person-centered care based on the unique needs of consumer

  • Explore treatment recovery options with the consumer, clarify goals and strategies, provide education and resources, and assist consumers in accessing treatment and community supports

  • Participate as an active care team member, work with the care team to establish treatment goals as part of the comprehensive care plan for the consumer, and support the consumer in accessing those services

  • Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources

  • Provide RSN services to the consumer seven days per week, directly or on an on-call basis, including outside of business hours via telephonic coverage

  • Provide services on a mobile basis to consumers in any setting that is safe for the consumer and staff

  • Maintain professional relationships and connections with community resources and partners

  • Engage and build rapport with consumer, assist with referrals and coordination to services and resources including but not limited to primary care, mental health treatment, addiction treatment, recovery support, employment specialists, housing specialists, DTA and other social supports

  • Remain knowledgeable about the substance use disorder service system. Link the consumer to treatment and recovery resources including but not limited to Acute Treatment Services, residential treatment, nutritional counseling, support groups, medicated assisted treatment and Narcan education

  • Helps peers to manage crises and strives to create safe spaces when meeting with peers

  • Values communication and follows laws and rules concerning confidentiality, 42-CFR and respects others’ rights for privacy

  • Maintain record-keeping as required by CFS, funding sources, and accreditation authorities

  • Participate in all supervision and training activities as required by CFS, funding sources, and accreditation authorities
