Job summary

The Southeastern Council on Alcoholism and Drug Dependence, Inc. (SCADD) is seeking a compassionate Case Manager in New London, CT.

This full-time professional role involves assessing client needs, coordinating services, and guiding individuals through treatment and into community supports that sustain recovery. You will collaborate with counseling staff and external providers in a multidisciplinary team, conducting intake, referrals, and discharge planning while promoting trauma-informed,

Qualifications

• Strong communication, organizational, and problem-solving skills.
• Experience with motivational interviewing, crisis intervention, and community resource coordination preferred.

Responsibilities

• Interview clients, complete intake information, review records and confer with professionals to evaluate needs.
• Use motivational interviewing to coach and empower clients toward positive outcomes.
• Refer clients to other support services including medical evaluation, social services, and employment services.
• Plan, develop, and implement intervention plans that support a continuum of care and outcomes.
• Assist clients with treatment plan objectives through exploration of problems and decision making.
• Coordinate case management needs, bringing together services, agencies, resources, and clients for discharge planning and referrals.
• Provide crisis intervention services for clients in distress.
• Educate clients and groups on alcohol and drug use disorders and available resources.
