Job summary

Spectrum Health Systems, Inc. is seeking a Recovery Support Navigator to engage and support patients seeking treatment in the community, beginning pre-release and continuing after release from Gardner, MA and Concord, MA facilities.

Responsibilities include managing a caseload, documenting services for regulatory bodies, coordinating re-entry packages, and facilitating discharge planning for continuation of SUDT services.

Qualifications

• High school diploma required.
• Bachelor's degree in human services or related field preferred.
• Case management experience strongly preferred.

Responsibilities

• Provide non-clinical services to engage, educate and support patients seeking treatment upon release and their families.
• Begin pre-release support that continues after release.
• Manage an assigned caseload and complete required documentation for services per Spectrum, DOC and regulatory bodies.
• Discharge planning that supports initiation and/or continuation of SUDT services.
• Coordinate a re-entry package and document receipt of nasal naloxone.
