Job summary

Grace Community Care & Homes is developing its Addiction Treatment and Recovery department and seeks an Addiction Treatment Navigator to support person-centered services, reliable operations and accurate documentation. The role will coordinate treatment engagement, appointments, barriers, resource linkage and follow-up within an approved OBAT or OORP model.

It is a part-time, on-site or community-based position in New Jersey, with scheduling discussed during hiring.

Responsibilities

• Support treatment engagement, appointments, barriers, resource linkage and follow-up within an approved OBAT or OORP model.
• Collaborate with department leadership, intake, credentialing, billing and quality teams; maintain confidentiality and communicate service gaps.
• Participate in orientation, competency review and quality improvement.
