Job summary

Grace Community Care & Homes seeks an Addiction Treatment Navigator to support engagement and barrier removal within an approved office-based treatment model. The role emphasizes confidentiality, timely communication of service gaps, and collaboration with leadership and support teams.

This planned, part-time position in New Jersey will begin upon program approvals and staffing readiness, with on-site assignments as applicable and a focus on person-centered services and accurate documentation.

Qualifications

• Eligibility to meet OBAT or OORP navigator pathway requirements.
• Regulatory eligibility verified before appointment.

Responsibilities

• Support treatment engagement, appointments, barriers, resource linkage and follow-up within an approved OBAT or OORP model.
• Collaborate with department leadership, intake, credentialing, billing and quality teams; maintain confidentiality and communicate service gaps promptly.
• Participate in orientation, competency review and quality improvement.
