Job summary

Envision Unlimited is seeking a Mental Health Direct Support Professional - PRN to join a multidisciplinary team in Chicago. You will supervise and train clients, implement treatment plans, and support daily living activities in a compassionate, recovery-oriented setting.

Qualifications

• Be at least 21 years old.
• Pass a criminal background check.
• Valid Driver's license required.

Responsibilities

• Supervise and train clients as part of the multidisciplinary team and treatment plans.
• Facilitate client activities and complete documentation per DHS standards.
• Supervise the self-administration of medications and coordinate with medical staff as needed.
• Document services and notes on client ITPs and required logs.
• Maintain safe, clean living arrangements and assist with daily living tasks.
