Qualifications

• Bachelor’s degree in social work or related field or closely related degree.
• 1 year of experience with individuals with mental health, substance use, and/or co-occurring needs preferred.
• Meet all agency background checks, driver’s record, and drug screen requirements.
• Valid driver's license, automobile, and adequate insurance as required.

Responsibilities

• Utilize a trauma informed approach when assessing and coordinating support and services.
• Respond to referral sources including self-referrals, probation and parole agents, and case managers to complete screens and assessments.
• Coordinate follow-up services to link individuals to community resources or Milwaukee County services.
• Complete all forms in Avatar by required deadlines (consents, PPS NOMS, assessments, referrals, crisis notes).
• Collaborate with individuals, natural supports, and recovery team members to build a broader support network.
