Job summary

Wisconsin Community Services in Milwaukee, WI is seeking a Care Coordinator to provide intensive, individualized care coordination for diverse populations. You will partner with individuals, families, and providers to build and monitor recovery plans in a trauma-informed framework.

The role requires a Bachelor's in social work or related field, with preferred Master’s in social work and experience in behavioral health.

Qualifications

Bachelor’s degree in social work or related field.
1 year experience with mental health, substance use, and/or co-occurring needs preferred.
Meet all WCS and contractor requirements for criminal and caregiver background checks, driver’s record, and drug screen.
Valid driver’s license, automobile, and sufficient insurance to meet agency requirements.
Master’s degree in social work or related field preferred.
Experience working with Milwaukee County Behavioral Health Services preferred.

Responsibilities

Build trusting and respectful relationships with individuals.
Utilize a trauma informed approach when coordinating supports and services.
Collaborate with individuals, their natural supports, community resources, and the recovery team to develop, implement, and regularly update individualized recovery treatment plans.
Serve as a liaison with service providers to ensure coordinated care and quality service delivery; complete referrals.
Monitor progress towards self-identified goals and provide coaching and psychosocial rehabilitation.
Maintain regular communication with individuals through in-person visits, phone calls, and follow-ups.
Ensure access to transportation for program-related appointments.
Advocate for timely supports and empower individuals.
Respond empathetically to crises and setbacks.
Document interactions and update records in line with policies.
Complete all required trainings.
