Job summary

Qualifications

Responsibilities

• Complete clinical documentation (group notes, 30-day reviews, etc.).
• Perform case management tasks (schedule medical appointments, assist with food stamps, monitor medications).
• Participate in multidisciplinary treatment team meetings and provide group-based substance abuse recovery tools.
• Transport clients and, if required, related duties as part of patient care; may need to assist with a flat tire or drive state vehicle when needed.
