Zusammenfassung

Mental Health Association of Nassau County (MHANC) seeks a motivated Supportive Housing Counselor to assist adults with mental illness in Nassau County. You will provide supportive services, life skills coaching, and coordination with care teams to promote recovery and housing stability.

This full-time role requires a Bachelor’s in a Human Service field and at least 2 years of relevant experience; CPR and Narcan within 90 days; and reliable transportation to multiple locations.

Qualifikationen

• Bachelor’s degree in human services or a related field.
• Minimum of 2 years working with individuals diagnosed with a mental illness.
• Requires travel to various locations in Nassau County and reliable transportation.

Aufgaben

• Provide supportive services including daily living skills and linkage to community services.
• Conduct weekly visits during the initial four weeks after admission.
• Assist residents with financial documentation for recertification, leases, etc.
• Provide crisis management and coordinate care during hospitalizations.
• Assist in admissions/discharges and help with paperwork for entitlements.
• Act as liaison between participants, landlords and treatment team members.
• Conduct fire safety and disaster drills and ensure safe living areas.
• Complete reports in Electronic Medical Records and maintain timely documentation.
