Job summary

Bluegrass Community Health Center is seeking a Peer Support Specialist to join our health team in Lexington, KY. The role focuses on providing peer-led support, recovery education, and connection to social and medical services for patients in recovery from substance use.

Ideal candidates will bring at least 1 year of peer support experience and demonstrate strong communication, cultural competence, and the ability to work with the PCMH team to develop individualized recovery plans.

Qualifications

• Minimum education is high-school graduate or equivalent.
• Experience: 1 year in peer support preferred.
• Kentucky Peer Support Specialist certification/licensing is required.

Responsibilities

• Provide direct care services and peer counseling to patients.
• Maintain confidentiality of patient records and HIPAA compliance.
• Assist patients in developing recovery plans and linking to community resources.
• Document patient data and care activities in the chart accurately.
