CVS Health is seeking a Peer Support Specialist in Louisiana to guide and mentor members on their recovery journey. The role involves field-based visits, collaboration with care management, and coordination with healthcare providers to support personalized recovery goals.

Applicants should have 1–2 years in community/outpatient mental health or substance use settings, be RPSS-certified per Louisiana guidelines, and possess strong communication skills.

• Minimum 1–2 years in community/outpatient mental health or substance use setting.
• Certified Peer Support Specialist (RPSS) per Louisiana guidelines.
• Strong communication and interpersonal skills; travel up to 50%.

Responsabilidades

• Attend case rounds and assist team in identifying members who may benefit from peer support services.
• Educate care management on how peer support services aid recovery and improve access to care.
• Collaborate with healthcare professionals to align recovery goals with care plans.
• Assist care managers in developing person-centered plans and goals with members.
• May accompany members to medical or behavioral health appointments to support connections to resources.
• Conduct face-to-face or virtual visits and engage members in goal setting.
• Document non-clinical, person-centered notes about member outreach.
• Develop relationships with community resources to support members in remaining in the community.
