Health Here seeks a Family Nurse Practitioner to deliver primary and behavioral health services to adults in recovery from substance use and mental health disorders. You will conduct comprehensive assessments, diagnose conditions, and develop recovery-focused care plans.

You will prescribe and manage medications, including MAT, and provide ongoing care management while monitoring treatment outcomes. Collaboration with an interdisciplinary team will coordinate services and support quality

Qualifications

• Licensed Family Nurse Practitioner with current board certification.
• Experience with Medication-Assisted Treatment (MAT) and behavioral health care.
• Strong clinical judgment, patient assessment, and care-plan development.
