Job summary

Fairview Housing seeks a Family Nurse Practitioner to deliver medical care, health assessments, and medication management across its Lifestyle Recovery Programs in Southwest Virginia. You will collaborate with a treatment team to support recovery‑oriented care and ensure compliance with Virginia licensing and ASAM standards.

You will provide MOUD/MAT and psychotropic management, coordinate with hospitals and primary care, and maintain timely documentation in the EHR.

Qualifications

• Master's degree in Family Nurse Practitioner program.
• Current Virginia NP license with prescriptive authority.
• National board certification as Family Nurse Practitioner (AANP or ANCC).
• DEA registration (or ability to obtain prior to start).
• Practice authority per Virginia requirements (collaboration or autonomous practice).
• CPR/BLS certification.
• Clear background checks and TB screening; pre-employment drug screen.
• Two or more years of NP clinical experience.
• Strong assessment, diagnostic and prescribing skills; proficiency with EHR.

Responsibilities

• Complete medical history and physical assessments for new admissions.
• Assess withdrawal risk and determine level of care.
• Evaluate and treat acute and chronic conditions; order and interpret tests.
• Screen for infectious diseases and provide education or referral.
• Prescribe and monitor MOUD/MAT and psychotropic meds as appropriate.
• Review controlled-substance prescribing per Virginia requirements.
• Conduct medication reconciliation and monitor for interactions.
• Educate residents on medications and relapse-prevention supports.
• Participate in multidisciplinary treatment team meetings and care planning.
• Maintain accurate clinical documentation in the EHR; ensure HIPAA compliance.
• Coordinate with care teams and community providers; assist with discharge planning.
• Travel between program locations within the service area; on-call as assigned.
