Job summary

Placements USA LLC in Fitchburg, MA is seeking a Nurse Practitioner to lead and provide care in our office-based MAT clinic. You will work under the supervision of a physician and Health Center, focusing on prevention, screening, intervention and treatment for substance use disorders.

The role requires MA RN license, FNP certification, and a DATA 2000 Waiver to prescribe MAT, with experience in team-based, evidence-based care. Telemedicine capability is utilized to reach patients.

Qualifications

• Massachusetts RN license required; FNP certification in MA required.
• Current DATA 2000 Waiver to prescribe MAT.
• CPR/AED certification or equivalent.
• 3–5 years nursing experience preferred.

Responsibilities

• Provides direct patient care for MAT and related disorders, in-person and via telemedicine.
• Collaborates with the care team to integrate medical and support services.
• Prescribes medications and treatment plans per established protocols.
• Participates in CQI initiatives and quality assurance activities.
• Develops patient education materials and coordinates referrals.
