Arthur Center Community Health in Fulton, MO seeks a Community Support Specialist (Adult) to walk alongside adults with severe mental illness, helping build skills, confidence, and independence. Qualifications

• Bachelor's degree in a human services field.
• Associate degree with at least four years of relevant experience.
• High school diploma or GED with at least four years of relevant experience.
• Preferred experience in case management with adults and/or youth with severe and persistent mental illness.

Responsibilities

• Provide individualized services based on clients' strengths, needs, abilities, and preferences developed with client and family.
• Assist clients in completing Wellness or Treatment Plan goals and adjust as priorities shift.
• Educate and build skills in daily living, employment, education, symptom management, communication, budgeting, and housing for independence.
• Screen for suicide risk using appropriate tools.
• Collaborate with Health Care Home team to improve overall health outcomes.
• Provide services in community settings and assist with accessing services and resources.
• Discharge planning and communication with involved providers and families.
• Advocate for clients within systems and with service providers.
• Participate in on-call crisis intervention if QMHP.
