Job summary

Qualifications

• Bachelor's degree in sociology, social welfare, psychology or related field required.
• Registered nurse (RN) credential acceptable as alternative.
• High school diploma with 12 sem. credit hours plus two years’ mental health experience acceptable.

Responsibilities

• Complete a strength-oriented assessment and describe consumer problems.
• Manage case notes and input data into required systems within timelines.
• Coordinate housing, benefits, transportation, and employment services.
• Collaborate with families and community providers for recovery planning.
• Travel to client residences to deliver direct and community-based services.
• Support risk management and crisis planning as needed.

Education

Bachelor's degree in sociology, social welfare, psychology or related field

Registered nurse (RN)

High school diploma with 12 semester credit hours in sociology, social welfare, psychology or related field and two years’ experience in direct contact with mental health consumers
