Job summary

Highland Rivers Behavioral Health in Alabama is seeking an Addictive Disease Support Services Specialist to assist individuals with substance use disorders in building life skills, linking to needed resources, and transitioning back to the community.

You will work under ADSS supervision, participate in weekly team meetings, and help coordinate care with courts, DFCS, and other agencies while maintaining ethical relationships with clients and families.

Responsibilities

• Ensures individuals on caseload are seen at documented frequency per treatment plan and that release of information is in place.
• Assist individuals with development of interpersonal and community coping skills and resource linkage.
• Provides assessment, monitoring and self-management support to encourage recovery.
• Advocate with other agencies to secure resources for individuals.
• Provides assistance and support in crisis situations.
• Participates in weekly multidisciplinary treatment/recovery team meetings.
• Utilizes the multidisciplinary assessment to participate in the development of a comprehensive individualized service/recover plan.
• Works with ADSS supervisor/area manager; connects with community programs such as drug courts to bill accordingly.
• Documents contact attempts, ISP Reviews, and coordination of care with referral sources.
• Assist consumer in achieving objectives in their service/recovery plan.
• Establishes professional and ethical relationships with consumers, families, and coworkers.
• Transport consumers if needed.
