Job summary

AnMed in Anderson, SC is seeking a Care Coordination Discharge Planner to support adult and geriatric patients in the inpatient setting with discharge planning, financial counseling, psychosocial assessments, and community referrals.

The role collaborates with an interdisciplinary team, screens for post-discharge needs, educates patients and families on the discharge process, and connects patients to post-acute resources while ensuring regulatory compliance.

Qualifications

Bachelor’s degree in social sciences or related degree.
Knowledgeable regarding community resources.
Excellent communication skills.
Works in collaborative team approach.

Responsibilities

Creates a process for communicating and collaborating with inter-disciplinary team.
Screens patients admitted and assesses high post-discharge needs.
Educates patient and family on the discharge planning process and facilitates patient flow.
Arranges post-acute services and referrals to community agencies.
Maintains knowledge of resources and assists with financial counseling.
