Qualifications

• Associates degree required; Bachelor's in social or health services field.
• Experience in counseling field instruction/internship or practicum required.
• Licensure or certification in counseling as required by the state; CPR and de-escalation/restraint certification required.

Responsibilities

• Conduct group and/or family sessions under supervision and document outcomes.
• Co-facilitate sessions and assist with modifying problem behaviors.
• May perform initial and follow-up assessments for patients.
• May prepare individual treatment plans for assigned patients.
• Ensure timely, clear documentation of patient care and incidents.
• May act as a liaison between referral sources and patients.
• Provide crisis intervention to patients as needed.
• Plan for aftercare for assigned patients and liaise with insurance carriers.
• Provide case management to ensure individualized quality care.
• Evaluate patient needs and determine referrals to other programs or facilities.
• Demonstrate a positive, empathetic and professional attitude; address complaints promptly.
