Job summary

Sheppard Pratt is seeking staff for the Adult Residential Treatment Center in Towson, MD, to support adults with serious mental illness. You will assess client needs, create service plans, link clients to community resources and advocate for their welfare on a daily basis.

Additional responsibilities include enrollment assistance, coordinating appointments, medication management, entitlements, and careful documentation.

Qualifications

• High school diploma or GED and 1 year of related experience.
• Bachelor’s degree preferred.
• Willingness to obtain additional certifications and trainings as required.
• Driver’s license with 3 points or less and access to an insured vehicle.
• Availability to work days, nights, weekends and holidays as needed.

Responsibilities

• Assess and monitor client needs and develop service plans.
• Coordinate client medical appointments, transportation, housing, money management, etc.
• Develop and assist with client medication management plan.
• Obtain and maintain entitlements for clients.
• Maintain accurate and complete documentation.
• Develop and coordinate transitions and program discharge plans.
